The Conners’ Rating Scales 

These are a set of questionnaires used to assess symptoms of Attention Deficit Hyperactivity Disorder (ADHD) and other related issues in children and adolescents (aged 6 to 18).  They provide valuable insights from different perspectives:

 

Conners’ Parent Form

Parents complete this questionnaire to rate how often their child exhibits behaviors like inattentiveness, hyperactivity, impulsivity, and emotional problems.

 

Conners’ Teacher Rating Scale 

Teachers use this questionnaire to report on a child’s behavior in the classroom setting, providing another angle on potential ADHD symptoms.

 

Conners’ Self-Report

This self-assessment helps children and adolescents (typically aged 8 to 18) report on their own thoughts, feelings, and behaviors, offering a valuable perspective on their experiences.
